He should have been responding to his name before any other formal training began. It is hard to tell what the problem might be without being around the dog through his training. After sit, come should have been taught. Back up, teach him his name maybe by calling it and giving him a treat or something like that. After you are satisfied that he knows his name, teach come on lead and don't stop until he comes imediately every time called. These are only suggestions as I have never had either of these problems.
