a good mix will run you 103+ / yard if you walk in off the street. i know the math says 11...... yards but you find me a digger who will grade that perfectly to 4"...not gonna happen , id also put some deeper footings in areas of higher traffic around the edges . i would order 13 yards to compensate for sloppy digging/ grading and for "beefed up" areas.
other areas of added cost:
- added fiber in to concrete
- 4000 psi concrete instead of a 3000 mix
- form wood, metal keyway for control joints , steel mesh
- form/ grading labor
-finishing
also this is would be considered a small job so expect to pay more / sqft to get a crew to show up to do it
