If the existing paint is sound, then, I don't completely strip it. But, hook rash has usually taken it's toll on a hardbait if it has been thrown very much...whether it's caught fish, or not. The finishes on today's "manufactured" lures is not what it used to be. I just make sure there is no peeling paint and make sure all the edges are feathered. Sometimes, I spend 30 minutes just prepping a lure. But, why put all the effort into painting it if you're not going to do it right?
